Parent/Carer satisfaction survey

Solihull Council is asking all parents/carers of children and young people who have used the Short Breaks Activities services subsidised by Solihull Council to complete this Parent/Carer satisfaction survey.

Short Breaks is the term the Government uses to describe the opportunities provided for children and young people with an additional need or disability to take part in fun and enjoyable activities. Short breaks are also designed to give parents/carers a break from their caring responsibilities.

The Council wants to ensure we are receiving good quality services and value for money from the organisations we give money to. We would therefore appreciate an honest account of your views on the service received.

You do not have to answer any question you are not comfortable with and can answer as many or as few as you like.

You have been sent this survey as your son/daughter has attended a Short Break service in 2024.

 If you have more than one child with a disability who attends short breaks activities please complete a survey for each child.


Data Protection

The information collected in this survey will be used by the Children’s Commissioning and Placements Team at Solihull Council (childrenscommissioning@solihull.gov.uk) for the purposes of reviewing satisfaction with service provision in the borough. We may share the results of the survey with key partners for the purposes of service improvement.
